These are the forms an Armored Knight Technology Account Representative must send to a prospective affiliate. In addition, the Rep must request the Articles of Inc., TID#, and voided check. In addition we need to send the Noteworld form. The Armored Knight Technology Processor must ensure the file is complete and all documents were completed properly. Last Revised 01/31/2010

Branch Notworld Form

The Account Rep. or the Processor must fill in all of the company information, and the company split before sending to the affiliate branch. The Branch will need to add in the banking info and sign the bottom line.
